Please ask a member of our staff, and we will review our payment options to accommodate your financial needs. If special arrangements are needed, please talk to our office manager prior to receiving service.

Cancellation Policy

Your appointment is reserved especially for you. Should you need to cancel or change the date of your appointment, we require at least 24 hours notice as a courtesy to other patients seeking appointments. If less than 24 hours notice is given or if you miss an appointment, then a late cancellation fee or missed appointment fee of $50 for medical appointments and $75 for cosmetic appointments will apply.

In fairness to other patients, if you arrive late and we are not able to accommodate you on that day, a late cancellation fee of $50 for medical appointments and $75 for cosmetic appointments will apply.

For all cosmetic appointments, a guarantee of $75 will be required with a credit card number. This number will be kept in a secure location and the $75 fee will only be charged to your account if a 24-hour cancellation is not given or you do not show for your appointment.

Insurance

Additionally, we will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days.

Please note Midwest Dermatology does not accept Medicare or Medicare Advantage plans. When the following list states "All Products" this should be interpreted as all products aside from Medicare products.
