• Please call our office 913-317-5066 to set up your initial in-office or online appointment.
• We will call you a day or two prior to your appointment to confirm.
• If you need to cancel your appointment, we do require a 24-hour notice or late fees may be applied.
• New Patients please arrive around 10-15 minutes prior to your appointment for account set-up.
• New Patient Package forms are under “Patient Resources” to fill out prior or they’re available in office.

Please Bring to Appointment:

• Driver’s License
• Current Insurance Card
• Payment Card (HSA, Flex Spending, Visa, MasterCard, Discover, American Express, Debit)
• List of medications and supplements you are currently taking
• Preferred pharmacy name and location
• Referral if required by Insurance
